1. APP PERMISSIONS
(i). Permissions-
(a) Allow ownUrHealth™ to take pictures : At Sign Up, the Application uses your CAMERA permission to capture your photo and upload it to the Server to verify your identity.
(b) Allow ownUrHealth™ to access photos and media: READ_EXTERNAL_STORAGE/WRITE_EXTERNAL_STORAGE permission (Media and Files)is required to store your captured photo, video, audio or any file into your phone's storage memory and upload it to the server. Photo is required at Signup.
(ii) INTERNET, ACCESS NETWORK STATE and ACCESS WIFI STATE:
To perform network operations in an application for updating your data to Server. Operations like detecting if the internet connectivity is present or not (either wifi or Mobile Data).
(iii) Fingerprint
Application uses your device fingerprint functionality for user authentication to make it easy to login.
We don't share information or data with any other 3rd party organization.
By your consent, we will use the above permissions and data for the purpose of functionality required for the working of the ownUrHealth™ App.
While using Our Service, We may ask You to provide Us with certain personally identifiable information that can be used to contact or identify You. Personally identifiable information may include, but is not limited to:
Usage Data is collected automatically when using the Service.
Usage Data may include information such as Your Device's Internet Protocol address (e.g. IP address), browser type, browser version, the pages of our Service that You visit, the time and date of Your visit, the time spent on those pages, unique device identifiers and other diagnostic data.
When You access the Service by or through a mobile device, We may collect certain information automatically, including, but not limited to, the type of mobile device You use, Your mobile device unique ID, the IP address of Your mobile device, Your mobile operating system, the type of mobile Internet browser You use, unique device identifiers and other diagnostic data.
We may also collect information that Your browser sends whenever You visit our Service or when You access the Service by or through a mobile device.
While using Our Application, in order to provide features of Our Application, We may collect, with Your prior permission:

The personal information collected from you at the time of registration will be stored on the secured Server and only be used by the CHERI organization in anonymized, aggregated datasets for the purpose of generating reports. Our Service does not address anyone under the age of 13. We do not knowingly collect personally identifiable information from anyone under the age of 13. If You are a parent or guardian and You are aware that Your child has provided Us with Personal Data, please contact Us. TERMS AND CONDITIONS OF USE
These Terms and Conditions (“Terms”) govern your use of the online E-Visit service. These Terms apply in full force and effect to your online use of E-Visit and by using E-Visit, you expressly accept all terms and conditions contained herein in full. You must not use E-Visit if you do not agree with or have objections to any of the Terms.
Eligibility Requirement
You must be over the age of 18 and a current annual user of ownUrHealth App and have a MyChart Account. You must be a current patient of a registered Doctor of the ownUrHealth App and, or an affiliated Registered Referring Entity for the ownUrHealth App.
Nature of Service
The service allows you to electronically consult with your doctor or primary care provider online for common health issues. It is an alternative to a traditional office visit. All communication occurs in MyChart and Notifications, and Online. There is a video/visual or verbal contact between you and the provider. Your E-Visit will become part of your permanent medical record and will be available to those who participate in your care and treatment.
Health information provided by you is accessible to your doctor or primary care provider staff that assist your doctor in providing medical care or support services. Sending sensitive information that you only wish to discuss directly with your doctor may not be appropriate.
Type of Conditions/Symptoms Covered
E-Visits are to be used for non-urgent symptoms or routine medical concerns. An E-Visit is not meant to replace seeing your doctor for more serious health problems. You should contact your doctor directly to schedule an in-person appointment if:
You have an urgent condition;
You have a condition requiring a doctor examination or lab tests;
You are experiencing a higher-risk pregnancy; or
You have had a previous visit for the same condition within the past seven days.
Cost
Based on your insurance coverage or, the lack of an insurance coverage, you may be assessed a co-pay amount or direct bill for an E-Visit.
Risks of Technology
Electronic online communication technology has potential risks, including service interruptions and deficiencies in transmission which may impede the provision of care through an E-Visit. Despite the use of security measures and encryption by ownUrHealth App, any information transmitted by electronic technology, including medical information, could potentially be illegally accessed by unauthorized third parties.
Confidentiality of Medical Information
Your E-Visit will become part of your permanent medical record and will be available to those who participate in your care treatment. All existing laws regarding your access to health care information and copies of your health care records apply to an E-Visit.
MyChart Terms and Conditions
Because online access to E-Visit is through your ownUrHealth App MyChart Account, the MyChart by ownUrHealth App Terms of Use and Agreement supplement these terms and conditions and together govern the use of E-Visits.
